Case Management

 A collaborative process of assessment, planning, facilitation, care coordination, evaluation and advocacy for options and services to meet an individual’s and family’s comprehensive health needs through communication and available resources to promote patient safety, quality of care, and cost effective outcomes.

 

Community Choice Guides

Is an intensive one on one support services for individuals in qualified community settings. Services may include;

  • Pre-Transition Consultation with potential participants.

  • Liaison acting on behalf of HCS and AAA staff between nursing facility and in-home case management, staff at nursing facility and institutional settings, housing, medical personnel, legal representatives, formal caregivers, family members, informal supports and any other involved party

  • Locating and arranging for appropriate, accessible housing

  • Setting up the participant's new home including services such as arranging utility hook-ups and the acquisition of furnishings and household goods

  • Arrangement of the participant's physical move from the institutional setting

  • Liaison between the participant and housing provider

  • May include training, education and consultation to help participant to access community settings, or health services.

  • May connect participant with less formal or atypical daily activities, such as those that might be found through local organizations, or Community Centers.

  • Provision of assistance and support to ensure the participant's successful transition from the institutional setting.

Housing Locate and Placement

Find appropriate housing for qualified individuals who are needing community based housing. Housing options are adult family homes, assisted living facilities, group homes, skilled nursing facilities, and independent housing.

 

Transitional Planning for Special Needs

 Transition services are intended to help those with limitations make the transition from rehab to home placement. It is a collaborative process of involving care coordination and advocacy for positive outcomes.

Referral Consultations:One on one consultation with resident, family, and caseworkers

Home Site Checks and Tours: Provides tours to facilities and home site evaluations.

Care Conference Consultations: Care conferences are held for every person receiving health care at home or in a care facility. They help the “care team” ~ everyone involved in the person’s care ~ share information and work together to meet the person’s needs.

Facility Inspections: vital to the safe, efficient functioning of a facility. Basic inspection techniques include performing visual surveys and randomly operating fixtures, equipment and systems.
